Cardiology Patient Navigator – Remote Coordinator

About the role

Our client is looking for a Cardiology Patient Navigator to support a busy U.S.-based cardiology team. Working remotely, you will manage patient communication, scheduling, and care coordination while ensuring accurate clinical documentation.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows in EMR/EHR systems.
• Conduct telephonic or remote outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology pathways, including CHF management protocols.
• Track testing schedules, follow‑up appointments, and remote patient monitoring workflows for a caseload of 150+ patients.
• Escalate clinical concerns to the appropriate provider within scope (no diagnosing or prescribing).
• Maintain accurate clinical documentation and ensure HIPAA‑compliant records.
• Assist with billing documentation and insurance coordination as needed.

Required profile

• Clinical healthcare background (CNA, MA, RN, or equivalent).
• 3+ years of experience in healthcare, telehealth, or medical office settings.
• Strong patient‑first attitude, organized under pressure, and comfortable on the phone.
• Ability to manage a high‑volume, remote workload with warmth and clinical precision.
• Bilingual proficiency (B2+ English and B2+ Spanish) is required.
