Cardiology Patient Navigator

About the role

Our client is looking for a remote Cardiology Patient Navigator to support a busy U.S.-based cardiology team. The role focuses on patient communication, scheduling, care coordination, and clinical documentation, ensuring patients stay connected to their care plans.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows within EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology pathways, including heart‑failure management protocols.
• Track testing schedules, appointments, and remote monitoring workflows for a caseload of 150+ patients.
• Escalate clinical concerns to providers within scope (no diagnosing or prescribing).
• Maintain accurate clinical documentation and ensure HIPAA‑compliant records.
• Assist with billing documentation and insurance coordination as needed.

Required profile

• Clinical healthcare background (CNA, MA, RN, or equivalent).
• Minimum 3 years of experience in healthcare, telehealth, or medical office settings.
• Experience in care coordination or management, preferably in remote/telephonic environments.
• Bilingual proficiency in English and Spanish (B2+ level).
• Ability to handle a high‑volume, remote workload with strong organization and communication skills.

Required skills

• Proficiency with EMR/EHR systems.
• Experience using remote patient monitoring platforms.
